New Patients

Our office will obtain medical records at the time of your first appointment.

We participate with all major insurance plans such as Medicare, BCBS of GA, Cigna, etc.

We do not participate with Medicaid.

We encourage you to complete your MEDICAL HISTORY on the Mod Med PORTAL prior to your appointment.  Upon scheduling, you will get instructions on how to sign up for the PORTAL.  This will facilitate a fast & easy check in process. Please bring your insurance card and photo ID, as well as a medication list to your appointment.  This will allow our staff to best assist you in a prompt manner.
