New Patient Info

Our office will obtain medical records at the time of your first appointment.

We participate with all major insurance plans such as Medicare, BCBS of GA, Cigna, etc.

We do not participate with Medicaid.

We encourage you to complete your MEDICAL HISTORY on the Mod Med PORTAL prior to your appointment.  Upon scheduling, you will get instructions on how to sign up for the PORTAL via your e-mail.  This will facilitate a fast & easy check in process. 

Please bring your insurance card, photo ID and medication to your appointment.  This will allow our staff to best assist you in a prompt manner.
