Our office will do the work for you and obtain any medical records needed at the time of your first appointment.
We participate with all major insurance plans such as Medicare, BCBS of GA, Cigna, etc.
We do not participate with Medicaid.
We encourage you to complete your medical history on the Mod Med portal prior to your appointment. Upon scheduling, you will get instructions on the portal. This will facilitate a fast & easy check in. Please bring along a printed copy of your medication list , your ID and, Insurance card to your appointment. This will allow our staff to best assist you in a prompt manner.
For patients with out of network insurance plans we do have self pay options available on a case by case basis. Pricing varies from visit to visit depending on the length of the appointment and what services are provided.
